• First, middle and last name, including suffix
• First and last name of child attending IWHS, and the student's ID number
• Maiden name
• Street address, including city, state, zip and country
• Address type (home or business)
• Phone number
• Phone type (home, business or cell)
• Email address
• Email type (home or business)
